Medical Records

The Cardiac Center Medical Records Section maintains all cardiology patient medical records. This includes patients who are seen at our clinic, Creighton Saint Joseph Hospital and Bergan Mercy Medical Center, and any of our 17 Outreach sites.

Medical Record Retention-Each patient's medical record contains all information regarding each visit with our cardiologists. We even have information for our patients who were seen during our start up years in the early 1960s. Our cardiologists feel it is in the patient's best interest to have all the information available so that the best plan of care can be pursued for each patient. These records are also frequently accessed by family members (with the patient's permission, of course) to obtain pertinent medical history regarding hereditary factors.

Instant Record Access--Our clinical areas are not the only areas that contain state-of-the-art equipment. The Cardiac Center maintains a hard copy medical record as well as storing each patient medical record on an electronic Optical Imaging System. We have 14 Imaging Stations located throughout our facilities at 3006 Webster Street and Saint Joseph Hospital. This gives 24 hours, seven days per week accessibility of patient medical records to our cardiologists and staff with the push of just a few buttons.

Staff Responsibilities--The Medical Records staff consists of eight staff members. They process approximately 1,400 documents each day assuring that the patient's medical record is accurate and up to date. The staff members are experts in placing each document in the proper area of the chart for quick and easy access by our cardiologists and staff.

Records Request--We also process approximately 150 requests for medical records from insurance companies, physicians, and patients. It is our policy that the patient must authorize any release of information. These requests must be in writing and submitted at least one week prior to the date the information is needed. Emergency requests for information are handled on an individual basis. All requests for information should be mailed to the following address:

You may use our "Authorization to Release Medical Information" form. We do accept other institutions' release forms and hand written requests from the patient.
